Add indexing to allow persistent references to digits.

Separate granularity of the user interface from that of the persistence (Ma and Meesen DigitCollection). Make choices for both.

Replace index with identifier to be compatible with ATLAS terminology.

Impose the requirement that we use Ma and Meesen classes to be consistent with the other subdetectors.

This is a first pass at the requirements and design. Comments on the requirements and design issues are especially encouraged. Once these are agreed upon, the classes and their interactions can be fleshed out.

For definiteness, I have made choices for each of the design issues but these can still be changed.
